To qualify for your Vaccine Handler Card:

• Click on the following link & go through the training at: EZIZ
• Once all 5 chapters are completed at EZIZ download the following form & fill it out:
• Fax all 5 EZIZ certificates & registration information form to (509) 249-6606

A Vaccine Handler Card will be made & sent to your clinic  for you to sign.  If you send your card back, we will laminate it for you free of charge. 

Vaccine Handler cards are good for 2 years.

To renew your card:

• Return to the EZIZ website & retake chapters 4+5.
• Fax the EZIZ certificates & updated registration information form to (509) 249-6606
